Results of all self-monitoring programs as described in the Report of Facility Information, will be reported as follows: <br />Program <br />Reporting Frequency <br />1. A report of quantities and types of wastes received and product removed from the facility <br />Quarterly to the LEA <br />shall be provided to the LEA on a quarterly basis (showing monthly breakdowns). The <br />records shall indicate the quantities ton/cubic yards and include weight and record data. <br />Records shall be submitted to the LEA on the 15' day of the subsequent month. <br />2.
