1.0. SCOPE OF WORK <br />PROJECT DESCRIPTION: Furnish supervision, labor, equipment and material <br />to remove one (1) 350 -gallon capacity gasoline underground storage tank ("UST") <br />including the following services: <br />a) PERMIT: Submitted Application For Underground Storage Tank Closure <br />Permit to Garrett Backus, Ld. Sr. REHS, CUPA programs of San Joaquin County <br />Environmental Health Department. <br />b) INSPECTIONS: Coordinate Inspector Backus and Fire Chief Ramirez of <br />Mokelumne Rural Fire District to witness tank removal process including safety <br />procedures and soil sampling_ Provide minimum of 48-hour advance notification of any <br />request for inspection. <br />c) HAZARDOUS WASTE DISPOSAL: Obtained State of California E.P.A <br />temporary tracking number CAC002887381 for disposal of rinsate from triple rinse <br />process under uniform hazardous waste manifest. <br />d) UNDERGROUND UTILITIES: Delineated exclusion work area and <br />street curb with ("USA") using white spray paint. Obtained Underground Service Alert <br />(USA) ticket number W630600908 and requested locating and marking of underground <br />public utilities prior to commencement of work. <br />e) SITE SAFETY: Provide barricades and caution tape to identify exclusion <br />work zone for public safety as per health and safety practice codes_ Conduct "tail gate" <br />site safety meetings to identify and mitigate safety hazards prior to commencement of daily <br />work. Provide (2) approved 20 BC rated fire extinguishers, hard hats, protective clothing <br />and other personal safety supplies. <br />f) TASK 1: Transport hydraulic backhoe/loader, one -ton construction utility <br />truck and equipment trailer to and from the jobsite. <br />g) TASK 2: Excavate and prepare to remove (1) 350 -gallon gasoline UST. <br />NOTE: Neither lateral piping nor electrical is associated subject UST. <br />h) TASK 3: Triple rinse UST and pump rinsate into 55 -gallon D.O.T. approved <br />steel drums labeled haz for temporary storage of hazardous waste liquids pending disposal. <br />4 <br />
