WESTERN PUMP <br /> Petroleum & Lubrication Equipment Specialists <br /> 5. A workplace accident or accident involving a company vehicle when the accident results in <br /> personal injury. <br /> 6 . Physical symptoms indicative of drug or alcohol use. For example, slurred speech , tremors , <br /> drowsiness , pupils dilated or constricted , irritability, hyperactivity, general motor impairment, <br /> disorientation , or the smell of alcohol on the breath . <br /> 7. Arrest or conviction for a drug or alcohol related offense or the identification of an employee asthe <br /> subject of a criminal investigation into illegal drug possession , use , ortrafficking . <br /> 8 . Employee admission . <br /> 9 . Any other facts or circumstances which would cause a reasonable person to believe thatthe <br /> employee is in possession of, or under the influence of drugs or alcohol <br /> Upon establishing reasonable cause , the employee's supervisor will promptly detail in writing the <br /> circumstances that formed the basis of the determination that reasonable suspicion existed to warrant <br /> the testing . A written record will be made of the observation leading to a controlled substances <br /> reasonable suspicion test within thirty six hours of the observed behavior. Original documentation will <br /> be kept for at least one year. A copy of the documentation will be given to the employee upon request. <br /> Return to Duty <br /> If you have violated the prohibited drug and alcohol rules , you are required to take a drug and/or <br /> alcohol test before returning to safety-sensitive functions . You are subject to unannounced follow- up <br /> testing at least 6 times in the first 12 months following your return to active safety-sensitive service . <br /> Follow-up <br /> The amount of follow-up testing you receive is determined by a Substance Abuse Professional and <br /> may continue for up to 36 months. This means the Safety Abuse Professional will determine how <br /> many times you will be tested (at least 6 times in the first year) , for how long , and for what substance <br /> (Le , drugs, alcohol , or both) . Your employer is responsible for ensuring that follow-up testing is <br /> conducted and completed , and your employer may have a policy that all follow-up tests are collected <br /> under direct observation .
