OBSERVATION: The Spill Prevention, Control, and Countermeasure(SPCC) Plan does not adequately discuss procedures for reporting a discharge, information and procedures for reporting a discharge are not included within the Plan. -Page 13 of the SPCC Plan listed an incorrect phone number for California Office of Emergency Services.This was corrected on site. -Page 28 and 29 of the SPCC Plan does not list damages or injuries caused by the discharge and if evacuations were needed as a results of the discharge. REGULATION GUIDANCE: The SPCC plan shall include: (vi)Contact list and phone numbers for the facility response coordinator, National Response Center, cleanup contractors with whom you have an agreement for response, and all appropriate Federal, State, and local agencies who must be contacted in case of a discharge as described in § 112.1(b). (4)Unless you have submitted a response plan under§ 112.20, provide information and procedures in your Plan to enable a person reporting a discharge as described in § 112.1(b)to relate information on the exact address or location and phone number of the facility;the date and time of the discharge,the type of material discharged; estimates of the total quantity discharged; estimates of the quantity discharged as described in§ 112.1(b);the source of the discharge; a description of all affected media;the cause of the discharge; any damages or injuries caused by the discharge; actions being used to stop, remove, and mitigate the effects of the discharge;whether an evacuation may be needed; and, the names of individuals and/or organizations who have also been contacted. CORRECTIVE ACTION: Ensure the SPCC Plan to include injuries, damages and evacuations as part of the procedures for reporting a discharge. Submit proof of correction to the EHD. This is a repeat violation, Class II. 620 CFR 112.7(f)(1) Failed to train personnel on all discharge prevention details listed in this section. OBSERVATION: It appears that the oil handling personnel were not adequately trained, as required. Section 2.6 on page 11 lists placing a temporary cover over the drain inlet located 35 feet southeast of the fuel unloading ramp during any fuel unloading operations. Per Ralph Alonzo and Paula Jessup, a drain cover is not part of their usual fuel unloading operations and the facility does not have a cover for this drain REGULATION GUIDANCE: (f)(1)At a minimum,train your oil-handling personnel in the operation and maintenance of equipment to prevent discharges; discharge procedure protocols; applicable pollution control laws, rules, and regulations; general facility operations; and,the contents of the facility SPCC Plan. CORRECTIVE ACTION: Ensure adequate training to all oil handling personnel and submit a copy of the training log and training content to the EHD. This is a repeat violation, Class II.
