`San Joaquin Local <br />Forward, Inc. <br />Health District <br />San Joaquin County Permit #39 -AA -015 <br />Page 3 of 4 <br />I. CONDITIONS: (continued) <br />C. Specifications: <br />1. No significant change in design or operation from that described <br />in items "I" A, B & C of the "FINDINGS" section is allowed, except <br />for those changes which are required under the "CONDITIONS" portion <br />of this permit. <br />2. The operator will immediately commence and thereafter maintain a <br />log of special occurrences. <br />3. This facility may accept limited hazardous waste provided prior <br />approval is received by the California Water Quality Control <br />Board Central Valley Region and the San Joaquin Local Health <br />District, and all hazardous wastes are handled in accordance <br />with the provisions of Chapter 6.5 (commencing with Section <br />25100) of Division 20 of the Health and Safety Code and <br />regulations promulgated thereunder. <br />4. The operator shall submit to the San Joaquin Local Health District, <br />on or before July 1 of each year, a pre -winter preparation plan <br />for this disposal site. <br />D. Provisions: <br />1. This permit is subject to review by the San Joaquin Local Health <br />District and may be suspended, revoked or modified at any time <br />for sufficient cause. <br />2. The following provisions must be met: <br />a. Incoming records of weights or volumes to within 10% <br />accuracy shall be instituted by the site operator by <br />September 1, 1978, <br />b. Names, addresses, telephone numbers and other emergency <br />information shall be provided at the disposal site by <br />September 1, 1978, <br />C. Identification signs shall be provided at each point of <br />access to the disposal site indicating the name and <br />address of the site operator by September 1, 1978. <br />d. Adequate barriers to prevent unauthorized access shall be <br />provided around the perimeter of the disposal site by <br />September 1, 1978. <br />e. Internal roads used by commercial vehicles shall be maintained <br />in a safe condition so that vehicle access and unloading <br />can proceed safely during inclement weather conditions by <br />August 15, 1978. <br />f. Sanitary facilities and handwashing facilities shall be <br />provided at the site by January 1, 1979. <br />g. Safe and adequate drinking water shall be provided for all <br />site personnel by September 1, 1978. <br />
