Project Hazard Identification, Risk Assessment and Control (HIRAC) Form
29
GENERAL WORK ACTIVITIES
Potential Hazards
Initial Risk
Ranking
(R/Y/G)
Hazard Controls
(including PPE requirements)
Applicable Policy or
Procedures
Residual Risk
Ranking (R/Y/G)

Infectious diseases
prevalent in your
location (COVID-19,
general influenza,
etc.)
between personnel. Limit participants at the meeting to as
few as possible.
• If social distancing cannot be maintained, then utilize a face
covering (nose and mouth) to minimize potential spread of
the virus.
• Prior to conducting any client site visits call your client site
representative to determine if they have any specific
pandemic-related requirements, access restrictions, or other
guidance that SLR needs to follow.
• Self-check yourself and members of your team concerning
level of health prior to mobilizing and accessing client sites.
• Where possible arrange for site inductions / sign-in
requirements to be done remotely in advance of the site visit.
• Minimize stops on trips; stay out of large gatherings, avoid
meeting rooms or spaces that put you in close proximity to
other personnel.
• When conducting work activities organize them in such a way
as to provide for social distancing when possible. For
example, when conducting borings for soil samples organize a
load off area where the driller helper can place the split
spoon and where the geologist can retrieve it while
maintaining their social distancing. Wear gloves when
handling materials and change them out frequently.
• Wash your hands regularly while on site. Ensure hand
sanitizer is available and used.
• Wear gloves to avoid touching common surfaces. Discard
gloves appropriately prior to entering your vehicle on
completion of site work.
• If PPE is required; bring your own to avoid using common
equipment.
• Clean / sanitize all PPE before and after use.
