The following is an itemized list of underground storage tank violations that have not <br /> been addressed for TRACY SHELL as of March 10, 2023. <br /> Open violations from January 30, 2023 inspection <br /> Violation #209 - Failure to maintain a copy of the "Facility Employee Training Certificate" on-site. <br /> 1 )OBSERVATION The Facility Employee Training Certificates dated 1 /20/2022 and 1 /20/2023 did not provide the <br /> dates when employees assumed responsibility. <br /> REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1 , 2018, or within 30 days of hire if prior to October 1 , 2018. The Facility Employee Training <br /> Certificate shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #210 - Failure to provide initial training, train every 12 months, and have a trained employee <br /> present. <br /> OBSERVATION According to the designated operator monthly inspection reports, employee trainings were <br /> conducted on 3/17/2022 , 5/13/2022, and 11/27/2022. Employee training records were not found for these dates. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br /> individual performs the duties of a facility employees. The training shall include, but is not limited to: <br /> 1 . Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br /> evaluating knowledge for responding to emergencies, leak detection , monitoring alarms, spills and overfills. At least <br /> one of the facility employees present during operating hours shall have current training. <br /> CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on <br /> site, and submit a copy of the training records to the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 4 of 6 <br />
