December 12, 1986 <br />MW -12 <br />Project 116-1.5 <br />The metal vault appears to be secure even though the concrete surrounding <br />the vault is cracked. A concrete pad will be constructed around the vault <br />to insure that the vault remains supported. The ponded water inside the <br />vault will be removed during well sampling in the future to prevent water <br />from cascading over the top of the casing. A padlock will be installed on <br />the locking vault. <br />The casing is screened from 100 ft to 80 ft, with a slot size of <br />0.02 inches. The annular space surrounding the slotted interval was <br />backfilled w:iLb #4 aquarium sand. A bentonite plug was placed above the <br />sand, and the remaining annular space was filled with a concrete surface <br />seal. <br />MW -13 <br />Ground water samples were obtained from this well on November 10, 1986. <br />The results of analyses performed on the water indicate that surface <br />water has flowed :into the well.. This well will be one of the first wells <br />to be replaced. The well will. be properly closed in compliance with state <br />and local regulations. <br />MW -14 <br />A padlock will. be installed on the locking vault. This well will <br />eventually be replaced because of the excessive length of the screen <br />interval. <br />MW -15 <br />The ponded water inside the vault will be removed during well sampling in <br />the future to prevent water from cascading over the top of the casing. A <br />padlock will be ip..st.alled on the Locking vault. This w^1.1 will eventually <br />be replaced because of the excessive length of the screen interval. <br />MW -16 <br />The vault will be repaired, and a padlock will be installed. This well <br />will eventually be replaced because of the 30 foot screen interval. <br />MW's-3, 4,5,6 and 8 <br />These wells were closed by removing the casing from the borehole, and <br />pumping a cement grout with 15% bentonite into the hole using a slurry <br />pump.
