Complaint Investigation Summary
Report #. 5105
COMPLAINT ID: CO5ISMEIV Site Location: 0 MANILA RD
Activity Summary
Activity Date
Recorded by
05/10/2021 Prop 65, Emergency Resopnse Record, and site map. MURO
Called Sandeep Singh, 510-648-8878, to ask if he received the Notice to Abate
and discuss. Sandeep stated that the number of containers rinsed was actually
approximately 10 — 15, and not 200.
Phone call to Augustin Diaz and Barry Dagenbach to request photos they took of
the Responsible party dumping residuals od pesticides.
Received photos of the chemicals and a list of chemicals observed on-stie from
Barry Dagenbach. The list is:
compiled a list of materials that were at the dumping site:
Merivon (Epa #7969-310)- Fungicide
Agri-Mek SC (Active ingredient: Abamectin) (Epa #100-1351) Federally
Restricted*-Insecticide/Miticide
Envy Six Max (Active ingredient: Glyphosate) (Epa #89167-47-AA-89391)-
Herbicide
Escalate 87 (7% Phosphoric acid)- Fertilizer
Arrow 2EC (Epa #66222-60-ZA)- Herbicide
Shark EW (Epa #279-3242)- Herbicide
Gauntlet (Epa #7001-50559)- Adjuvant
Quadris Top (Epa #100-1313-ZA)- Fungicide
Danitol 2.4 EC Spray (Epa #59639-35)- Aracide/Insecticide/Miticide
Vexer (Epa #91234-62-AA-89391)- Insecticide
Potassium Acetate 0-0-29- Fertilizer
PHT Ad-Max 90 (Epa #7001-50537)- Adjuvant
Weevil-Cide Aluminum Phosphide empty canisters (California R estric te d)*
Fumigant for Rodents (Volatile with water).
I let Barry know that Sandeep Singh stated that the number of containers rinsed
were 10— 15, and not 200.
Barry responded that he estimates that 90 chemicals were observed on-site, but
was not certain how many containers were rinsed. He believes at least 30
containers were rinsed.
Forwarded Barry's email to Isabella Bencomo.
05/11/2021 Received an email from Isabella Bencomo confirming receipt of the photos. She
asked if we have a site map indicating where the bins were rinsed and where the
ditch exists relative to the property and the river.
Isabella believes there is high potential for infiltration and since the depth to water
is about 5 to 10 feet, there's also high chance of interaction with the San Joaquin
River.
05/12/2021 Isabelle Bencomo (Water Board) requested site maps indicating where the bins
were rinsed and showing the ditch relative to the property and the river. I
generated three site maps with arrows.
MURO
MURO
5105 rpt
