53 CA Health and Safety Code 25292.2; CCR 2711(a)(11) 2010007 - Submitted/maintained current Cert of <br />Financial Responsibility/mech of financial assurance (TCR 11). <br />OBSERVATION: Current financial responsibility documents were not found on site. <br />REGULATION GUIDANCE: A copy of current financial responsibility documents are required to be maintained <br />on site. <br />CORRECTIVE ACTION: A copy of the facility's financial responsibility documents was printed on site during the <br />inspection. This was corrected on site. <br />This is (Minor) Violation. <br />Response: Corrected at time of inspection <br />56 CA Code of Regulations 2716(a -e) 2030013 - DO inspections completed at least every 30 days and meet all <br />requirements (TCR 12). <br />OBSERVATION: Designated operator reports were not signed within the required time frame. <br />-Designated operator reports were observed without completion of section 6. <br />-Designated operator reports were observed without completion of section 5, when compliance issues in <br />section 3 were identified. <br />REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual <br />inspection, the designated operator shall sign and provide the owner or operator with a copy of the <br />inspection report. Within 72 hours of being provided a signed copy of the report, the owner or operator <br />shall provide a description of each corrective action taken or to be taken and shall sign and date the <br />report, acknowledging the identified compliance issues. <br />CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within <br />the required time frame in the future. <br />This is a repeat violation (Class 2) Violation. <br />Response: Revised designated UST operator visual inspection reports attached. <br />58 CA Code of Regulations 2715(c)(3), 2715(c)(4) 2010015 - Copies of Facility Employee Training Certificates <br />maintained. <br />OBSERVATION: The Facility Employee Training Certificate dated September 2023 listed incorrect INITIAL <br />training dates. <br />REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility <br />employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee <br />Training Certificate shall contain accurate information for all employees trained. <br />CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information <br />and submit a copy of the updated form to the EHD. <br />This is a repeat violation (Class 2) Violation. <br />Response: Revised Facility Employee Training Certificate attached. <br />
