Site Health and Safety Plan Page 33 <br />8.2 Initial Notification and Response <br />Any site worker who identifies and or discovers a hazardous substance spill or release shall <br />immediately notify the SSHO and report, to the best of their ability: <br />• Hazardous substance involved <br />• Location of the spill <br />• Estimated quantity of substance spilled <br />• Direction/flow of spill material, if applicable <br />• Any related fire/explosion incidents associated with spill <br />• Any related injuries <br />8.3 Spill Evaluation and Response <br />The Site SSHO is responsible for evaluating spells in determining the appropriate spill <br />response. When this evaluation is conducted, the spill area shall be isolated and <br />demarcated to the greatest extent possible. When an incidental release occurs, cleanup <br />personnel shall receive instructions in a pre-cleanup meeting as to spill conditions, <br />necessary PPE, response activities, decontamination, and waste handling. <br />The procedures of the Emergency Response section of this HASP are immediately <br />implemented when a spell is determined to require emergency precautions and action. If <br />necessary to protect those outside the cleanup area, notification of the appropriate local, <br />state, and federal authorities shall be made. <br />8.4 Post Spill Evaluation <br />The spill area shall be inspected to ensure the area has been satisfactorily remediated. The <br />use of surface and air samples shall be utilized in this determination as necessary or <br />recommended. The root cause of the spill shall be examined and corrective steps taken to <br />ensure the engineering, administrative, and other control measures shall be developed and <br />implemented. <br />A written post-spill response report shall be prepared at the conclusion of cleanup <br />operations. The report shall include, at a minimum, the following information: <br />• Date of spill/incident <br />• Cause of incident <br />• Spill response actions <br />• Any outside agencies or consultants involved, including their incident reports <br />• Recommendations for process improvements to reduce spill potential and future <br />operations. <br />
