Ensure <br /> that all monitoring equipment is maintained and located to detect a leak at the earliest opportunity. <br /> This was corrected on site. <br /> This is (Minor)Violation. <br /> 53 CA Health and Safety Code 25292.2; CCR 2711(a)(11)2010007-Submitted/maintained current Cert of Financial <br /> Responsibility/mech of financial assurance(TCR 11). <br /> OBSERVATION: Section A of the Certification of Financial Responsibility document was filled out incorrectly. <br /> REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, and upload the required documents. <br /> This is a repeat violation (Class 2) Violation. <br /> 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all <br /> requirements(TCR 12). <br /> OBSERVATION:The designated operator failed to check and note the correct test dates for the Secondary <br /> Containment Testing on the designated operator inspection reports. The test date was recorded as 7/12/2024. The <br /> correct date is 7/23/2024. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br /> required information on the reports. <br /> OBSERVATION: On February 2025 the designated operator documented N/A to the questions "alarms were <br /> responded to appropriately"and the question about Containment sumps that alarmed was responded to by a qualified <br /> service technician. They should have been designated Y (yes). <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the <br /> alarm history with documentation taken in response to any alarms to the inspection report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br /> required information on the reports. <br /> This is a repeat violation (Class 2) Violation. <br /> 59 CA Code of Regulations 2712(b)(1-2)2030001 - Records of release detection monitoring and maintenance properly <br /> maintained (TCR 9d). <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site.
