Injury and Illness <br /> Prevention Program 2.22 <br /> The results of the audit/observation are entered into our internal digital application so that trends can be tracked <br /> and communicated throughout Roux to ensure that the corrective actions are effective and implemented. Once <br /> these digital forms are submitted, immediate notifications will be sent to the OHSM and CHSD. The OHSM, in <br /> coordination with the CHSD, assigns the severity level, which determines the appropriate response in order to <br /> provide a resolution and verify and validate that the solutions are effective. <br /> In addition to scheduled inspections and ongoing review, the CHSD, OM, and OHSM will arrange for unscheduled <br /> inspections. The times and locations are to be chosen by the person performing the inspection. <br /> Inspection Records <br /> Inspection records shall be maintained for a minimum of one year following the inspection. <br /> Hazard Correction <br /> All unsafe or unhealthy work conditions, practices, and procedures shall be corrected in a timely manner based on <br /> the severity of the hazards. Hazards shall be corrected according to the following procedures: <br /> • When observed or discovered <br /> • When an imminent hazard exists, that cannot be immediately abated without endangering employee(s) <br /> and/or property, we will remove all exposed workers from the area except those necessary to correct the <br /> existing condition. Workers who are required to correct the hazardous condition shall be provided with the <br /> proper training and necessary protection to perform the work to the safest extent. <br /> All employees are empowered to share their hazard identifications in the field through regular communication with <br /> their PM and through a digital form submission. Electronic submittals can provide real-time data to not only their <br /> PMs but also the OHSM and CHSD regarding the specific activity type, type of hazard identified, whether or not <br /> Stop Work authority was implemented, a brief description of the hazard, and the mitigating actions that were <br /> implemented to reduce the potential for an incident. <br /> 6. ACCIDENT INVESTIGATION <br /> It is Roux's policy that all work-related accidents, injuries,and illnesses are to be conducted in a professional manner <br /> to identify probable causes, and these are to be used to develop specific management actions for the prevention of <br /> future accidents. Every minor or non-disabling injury will be investigated with the same vigor and thoroughness as <br /> serious injuries. Proper and complete investigation of these injuries can be an effective injury prevention tool. <br /> All Roux employees shall report all incidents as soon as possible to the PM (or Administrative Manager for office- <br /> related incidents), Site Health and Safety Officer(SHSO), OHSM, OM, and CHSD, regardless of severity.Additional <br /> notifications to the COO and CEO shall be made based on the severity of the incident. <br /> On receiving a report of an incident (or"near-loss") occurrence from a Roux employee, the SHSO or OHSM shall <br /> immediately investigate the circumstances and shall make appropriate recommendations to prevent recurrence. <br /> The Incident Report form can be found in Appendix C, and the Near Loss form can be found in Appendix D. <br /> Management shall participate in the investigation of more serious accidents and incidents that occur on-site. The <br /> CHSD shall also be immediately notified by telephone on the occurrence of a serious accident or incident. <br /> Details of the incident shall be documented using the Accident Report and Investigation Forms within twenty-four <br /> (24) hours of the incident and shall be distributed to the SHSO, the OHSM, PM, OM, and the CHSD. The CHSD <br /> will update OSHA Forms 301 and the 300 log when necessary. <br /> 03/2025 Corporate Health and Safety Management Program I ROUX 15 <br />
