Injury and Illness <br /> Prevention Program 2.22 <br /> A satisfactory accident report shall answer the following questions: <br /> 1. What happened?The investigation report should begin by describing the accident, the injury sustained,the <br /> eyewitnesses, the date, time, and location of the incident, and the date and time of the report. Remember: <br /> who, what, when, where, and how are the questions that the report must answer. <br /> 2. Why did the accident occur?The ultimate cause of the accident may not be known for several days after <br /> all the data is collected and analyzed. Roux shall formally go through the Factors, Root Cause, and <br /> Solutions Form following an accident to determine the main cause. <br /> 3. What should be done? Once a report determines the cause of an incident, it should provide a means and <br /> method for mitigating future incidents of similar character. This is a decision made by the CHSD, PM, and <br /> upper management. Once a solution has been adopted, it is everyone's responsibility to implement it. <br /> 4. What has been done? Verification and validation will take place of the solution. In the first step, one will <br /> verify that the solution has been implemented, and the second action will determine whether the solution <br /> that has been implemented works to reduce the likelihood of a reoccurring incident. <br /> 7. TRAINING AND INSTRUCTION <br /> All workers, including managers and supervisors, shall have training and instruction on general and job-specific <br /> health and safety practices. Training instruction shall be provided: <br /> • When the IIPP is first established <br /> • To all new employees <br /> • To all employees given new job assignments for which training has not previously been provided <br /> • Whenever new substances, processes, procedures,or equipment are introduced and present a new hazard <br /> • Whenever the employer is made aware of a new or previously unrecognized hazard <br /> • To all workers with respect to the hazards specific to each employee's job assignment <br /> Employee Training Documentation <br /> Roux maintains health and safety training documentation for each employee, including employee name, training <br /> dates, type(s) of training, and training providers (as applicable). Documentation of training will be maintained for <br /> three (3)years. <br /> Training records of employees who have worked for less than one (1) year for the company will not be retained <br /> beyond the term of employment if they are provided to the employee upon termination of employment. <br /> Training records are maintained at the office level with the OHSM, as well as within a Corporate Training Database. <br /> 8. RECORDKEEPING AND DOCUMENTATION <br /> Copies of required accident investigations and certification of employee training shall be maintained at the office <br /> level with the OHSM, as well as within a Corporate Database. <br /> Roux keeps records of its employee fatalities, injuries, and illnesses that are work-related, are a new case, or meet <br /> one or more of the general recording criteria of 29 CFR 1904 Recording and Reporting Occupational Injuries and <br /> Illnesses and Title 8 Sections 14300-14300.48. <br /> Each recordable injury or illness is entered on the OSHA 300 Log of Work-Related Injuries and Illnesses, OSHA <br /> 301 Form Injury and Illness Incident Report, and a separate, confidential list of privacy-concern cases, if any, within <br /> 03/2025 Corporate Health and Safety Management Program I ROUX 16 <br />
