Provide Subcontractor Oversight <br />During Lift and Load Placement <br />- Unqualified Operator <br />- Falling Objects <br />- Heavy equipment hazards could be <br />present <br />- Lifting equipment (cranes) <br />- Personal Injury <br />- Struck by Falling Load <br />- Inadequate communication <br />- Unauthorized Entry <br />- Moving parts of crane <br />- Faulty Equipment <br />- Line-of-Fire <br />1. Review operator and rigger's credentials onsite prior to lift. <br />Operator and rigger must have a valid certification. <br />2. Ensure crane operator and qualified rigger has reviewed the <br />lift plan and documented inspections of rigging has been <br />completed prior to lift. Rigging must be rated for load as <br />described in subcontractor lift plan. <br />3. All other heavy equipment operations not associated with the <br />lift must cease prior to lift, if activities are within 200 feet of the <br />drop zone. <br />4. Non-essential personnel shall not be permitted within 75' feet <br />of outer perimeter of drop zone established by subcontractor. <br />Non-essential personnel are individuals not assigned to the task <br />nor have permission from the site supervisor or foreman. <br />5. Mental Risk Assessments (MRAs) and Activity Hazard Analysis <br />(AHAs) tool must be utilized to help mitigate risks to prevent <br />personal injury and property damage. Discuss "What could go <br />wrong?" and What's the worst that could happen if something <br />does go wrong?". Do not proceed if anything is unsafe. <br />6. Ensure nobody stands beneath the fall or crush zone or in <br />direct line-of-fire of a suspended load. <br />7. Identify final resting place of load prior to lift so as to avoid <br />any confusion while lift is underway. Ensure Crane Operator and <br />spotter have clear line of sight with one another and both are <br />communicating using proper hand signals or radios. <br />8. No ICS/Client or other personnel is authorized to enter the <br />subcontractor (Bragg Crane and Rigging) exclusion zone during <br />lift. <br />9. Moving parts of crane such as spool, outriggers, boom, and <br />path of travel are extremely dangerous. Ensure personnel no <br />personnel are located within the exclusion zone, which is within <br />the swing radius of crane/load. <br />10. Verify that equipment inspections have been conducted and <br />that work Stops if/when it becomes broken or compromised in <br />any way. (i.e. frey cables onspools, damaged shackles, etc.) <br />11. Check that swing radius of boom is not in direct path of <br />people or property. Operator must keep boom of crane at least <br />20' feet away from structures at all times and powerlines that are <br />350kV, per OSHA 1926.1407. <br />12.
