<br />8 <br /> <br />6) Inspection of jobsites for adherence to the Code of Safe Practices, company <br />policy, and regulatory requirements. <br />Inspections will be documented and maintained in accordance with California Code of <br />Regulations (CCR) Title 8. <br />Inspections will be conducted at the following intervals, in addition to those times <br />mentioned above: <br />At the effective date of this program. <br />Whenever new substances, processes, procedures, or equipment are introduced to <br />the workplace, they represent a new occupational safety and health hazard. <br />Whenever ICS is made aware of a new or previously unrecognized hazard. <br />The following approaches will be used periodically to evaluate the workplace further. <br />• Checklists <br />• Jobsite inspections conducted by the ICS Health and Safety Director or a <br />designated individual. <br />• Records Review <br />• Employee Suggestion Box <br />• Inspections of the jobsite by the Safety Committee <br />VI. Accident, Injury, and Illness Investigations <br /> <br />Accidents are investigated by the Health and Safety Director and Site Safety Officer <br />using a standard form. Reports identify causes and corrective actions. Findings are <br />reviewed by management and the Safety Committee. <br />When accidents, injuries, or illnesses occur on the job that require medical attention, <br />The Health and Safety Director and the Site Safety Officer will thoroughly investigate <br />them. <br />Investigators will complete the Injury/Illness Investigation Form (see Appendix E) <br />to determine the following: <br />2) Who and what were directly involved in the accident? <br />3) Who and what were indirectly involved in the accident? <br />1) Where and when did the accident occur?
