SAN JOAQUIN AMENDED Environmental Health Department <br /> COUNTY— 1/8/2026 <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> GAVINKO PETROLEUM* 17700 MORELAND ST, STOCKTON TOctober 16, 2025 <br /> 5 2632(d), 2641(h) 2030041 -A current facility plot plan is maintained and has been submitted <br /> OBSERVATION: An accurate UST Monitoring Site Plan was not submitted. <br /> -The UST Monitoring Site Plan currently indicates that all three Leak Detectors on site are a "LD2000"this is incorrect, it <br /> is a 8484 or it may be listed as 848480-001. <br /> -The location of the monitoring panel is not indicated in the map <br /> REGULATION GUIDANCE: A site plan must be submitted identifying the locations where monitoring will be performed. <br /> The site plan must show the general layout of tanks and clearly identify locations of the following equipment, if installed: <br /> 1) monitoring panels; 2) in-tank liquid level probes (if used for leak detection); 3)devices monitoring tank annular spaces <br /> or vault; 4) devices monitoring product piping; 5)devices monitoring fill piping; 6)devices monitoring vent piping; 7) <br /> devices monitoring vapor recovery piping; 8) devices monitoring vent/transition sumps; 9) devices monitoring under- <br /> dispenser containment; 10) line leak detectors; and 11)devices monitoring any other secondary containment areas. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a Repeat(Class 2)Violation. <br /> 7 2715(a)(1)(113), 2715(a)(3) 2010003 - Owner/Operator submitted DO form and/or informed the CUPA of any change(s) <br /> within 30 days (TCR 10) <br /> OBSERVATION: A new designated operator, Eric Meeker, began conducting inspections on 5/9/25 and notification was <br /> not provided to the EHD within 30 days of the change. <br /> REGULATION GUIDANCE: The owner or operator shall submit a "Designated Underground Storage Tank Operator <br /> Identification Form" no later than 30 days after any change in the designated UST operator(s). <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br /> This is a Repeat(Class 2)Violation. <br /> EHD-CUPA Inspection Report IS03 Page 5 of 7 FA0003732 PR0231819 4565085 1/8/2026 <br /> CP22-UST FULL g <br /> 1868 E.
