The following is an itemized list of CP22 - UST FULL violations that have not been <br />addressed for Harney Lane 76 as of January 15, 2026. <br />Open violations from July 17, 2025 inspection <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #7 - 2010003 - Owner/Operator submitted DO form and/or informed the CUPA of any change(s) <br />within 30 days (TCR 10). <br />OBSERVATION: Designated Underground Storage Tank Operator Identification Form was not submitted on CERS . <br />REGULATION GUIDANCE: The owner or operator shall submit a "Designated Underground Storage Tank Operator <br />Identification Form" no later than 30 days after any change in the designated UST operator (s). <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #10 - 2030074 - Annual Monitoring System Certification Form to the CUPA within 30 days of <br />completion of the test. <br />OBSERVATION: Annual monitoring system certification and leak detector testing was performed on 7/25/2024 and a <br />test report was submitted on 10/10/2024, 47 days late. <br />REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the tests. <br />CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are performed <br />in a timely manner. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Page 2 of 7
