5.2 The hazard assessment can also be performed by reviewing the JSA or TBJSA (APPENDIX A), and <br /> certifying that the hazard assessment has been completed by signing and dating the Hazard <br /> Assessment Matrix(APPENDIX D) that has been customized to location specific-jobs and hazards. <br /> IMPORTANT: Please note that the Hazard Assessment Matrix downloaded from the Safety& Health <br /> website is a starting template that must be customized to the location's job positions and hazards <br /> assessed for those job positions at that location. <br /> 5.3 The Hazard Assessment Matrix must be signed and dated by the GM. <br /> 5.4 The Hierarchy of Hazard Controls will be used to control or mitigate identified potential hazards. <br /> 1. Elimination of the hazard (Prevention Through Design) <br /> 2. Substitution of less hazardous materials, processes, operations, or equipment <br /> 3. Engineering controls - using limit switches, interlocking systems, two-hand operation and/or guard <br /> the hazard using rigid and permanent materials. <br /> 4. Warnings -awareness warning devices such as alarms, horns, buzzards, bells, and lights, use of <br /> warning signs and barriers <br /> 5. Administrative controls— use of safe working practices, procedures, policies, programs, SOP's, <br /> authorization, and supervision. <br /> 6. Personal protection equipment(PPE)—using safety glasses, hard hats, bump-caps, FRC, <br /> hearing protection, safety toe footwear and etc. <br /> 5.5 Besides step one, eliminating any combination of the other elements can be used together to <br /> provide extra safety protection. <br /> 5.6 A localized PPE Inventory Checklist(APPENDIX B) must be completed monthly confirming the <br /> required PPE inventory is on hand at all times. (PPE requirements are based on the current JSA for <br /> that position). <br /> 5.7 PPE Inventory list must be kept at each PPE station and adequate supplies of PPE, as identified on <br /> PPE Hazard Assessment, must be stocked and maintained. Centralized PPE station must be <br /> provided. (At FAS locations, inventory/stock cannot be used to meet this requirement). <br /> 5.8 All locally identified hazards and PPE have been added to the location JSA. <br /> 5.9 A Task-Based Job Safety Analysis (TBJSA) should be a findings or event driven process as <br /> deemed necessary by the location and must be reviewed and approved by a member of the <br /> corporate safety team. Event driven examples include: <br /> 1. When a new job task is created that may require specific-safety steps <br /> 2. When an injury incident or near miss occurs <br /> 3. When a Partner has concerns about the safe job steps <br /> 5.10 Affected partners must indicate that they have reviewed and understand all JSA's and Task-Based <br /> JSA's that are applicable to their job position, whenever their job or task changes, whenever a JSA <br /> or TBJSA is updated, and at least annually by signing the appropriate JSAs, TBJSAs, or the JSA <br /> Acknowledgment Form for Multiple Partners- Multiple JSAs (Appendix C). <br /> 5.11 Affected partners must sign documentation indicating that they have been trained in how to use the <br /> PPE and that the partner"understood" the training by signing the JSA/PPE Assessment at the <br /> completion of the training. <br /> 6.0 General Safety and Housekeeping <br /> 6.1 Metal slivers, lint and other debris should be cleaned from all areas using a brush or rag.
