Hazardous Materials Business Plan Program Inspection Report <br />Date: <br />February 3, 2026 <br />Facility Address: <br />505 S MAIN ST, LODIM&R CO <br />Facility Name: <br />Environmental Health Department <br />10 HSC 6.95 25506, 25505(a)(1), 25508(a)(1), 25508(a)(3) 1010004-Complete and accurate Hazardous Materials Inventory <br />information electronically submitted <br />OBSERVATION: The business failed to electronically submit complete and accurate chemical inventory information for <br />all reportable chemicals onsite. The following deficiencies were found during the inspection: <br />1- Eight 55-gallon barrels of Peroxyacetic Acid (PAA) observed onsite and only two 55- gallon (1,042 pounds) reported to <br />CERS. The Eight 55 -gallon barrels were observed onsite at the following locations: <br />a. Two 55-gallon barrels by the Main Shed Hydro-cooler/HC-4 <br />b. Three 55-gallon barrels by System 1 South Hydro-cooler <br />c. One 55- gallon barrel outdoor by System 1 Condenser. <br />d. One 55- gallon barrel by Export Shed Hydro-cooler 1 <br />e. One 55- gallon barrel by Inline hydro-cooler <br />2- The following reportable chemicals were not reported to CERS: <br />a. One 55-gallon barrel of Sani 10% located by System 1 Main Shed Hydro-cooler <br />b. One 55-gallon barrel of Sani 10% located by Export Shed Hydro-cooler <br />c. One 55-gallon barrel of Alpet D2 located by System 1 Main Shed Hydro-cooler <br />d. One 55-gallon barrel of Cleaner L-130F located by System 1 Main Shed Hydro-cooler <br />3- The following hazardous material inventory forms have inaccurate information: <br />a. The Hazardous Material Type for PAA reported incorrectly as “Pure". The labels on the chemical containers indicate <br />the chemical is a mixture with 15% or 5.6 % of PAA. <br />b. The reported storage pressures and storage temperatures entered for Ammonia are incorrect. <br />c. The reported storage pressure for Methyl Bromide is incorrect. <br />d. The chemical location for compressor oil and compressor oil waste reported incorrectly as South of the building. <br />These chemicals are located inside the export machinery room/North of the building. <br />REGULATION GUIDANCE: An accurate and complete hazardous materials inventory shall be submitted by the due <br />date. <br />CORRECTIVE ACTION: Submit complete and accurate chemical inventory information for all hazardous materials on <br />site at or above reportable quantities. <br />This is a Repeat (Class 2) Violation. <br />13 HSC 6.95 25505(a)(2), 25508(a)(1), 25508(a)(3) 1010005-Site Map with all required content electronically submitted <br />OBSERVATION: The business failed to electronically submit a site map with all required content. <br />The following chemical locations were not included on the site map: <br />a. Ammonia vessels with reportable quantities of Ammonia were not included on the site map. These include 2 <br />accumulators for system 1, 1 Accumulator for the Palletizing Room, 1 Accumulator inside the Main Shed, and 2 <br />Accumulators inside the Export Room. <br />b. One 55- gallon barrel of PAA by Inline hydro-cooler <br />c. One 55-gallon barrels of Sani 10% located by System 1 Main Shed Hydro-cooler <br />d. One 55-gallon barrels of Sani 10% located by Export Shed Hydro-cooler <br />e. One 55-gallon barrel of Alpet D2 located by System 1 Main Shed Hydro-cooler <br />f. One 55-gallon barrel of Cleaner L-130F located by System 1 Main Shed Hydro-cooler <br />g. One 55-gallon drum of waste oil (Compressor Oil Waste) located inside the Export machinery room <br />REGULATION GUIDANCE: A site map shall contain north orientation, adjacent streets, access and exit points, <br />evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br />present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map <br />requirements the governing body of the unified program agency finds necessary. <br />CORRECTIVE ACTION: Electronically submit a site map with all required content. <br />This is a Repeat (Class 2) Violation. <br />1868 E.
