SITE-SPECIFIC HEALTH AND SAFETY PLAN FOR PHASE II ENVIRONMENTAL SITE ASSESSMENT <br /> Emergency Response <br /> July 7, 2023 <br /> • If the driver of the vehicle is not familiar with directions to the hospital, a second person shall <br /> accompany the driver and the injured employee and navigate the route to the hospital. <br /> • If it is necessary for the SHSO to accompany the injured employee, provisions will be made to <br /> have another qualified employee, properly trained and certified in First Aid, to act as the <br /> temporary SHSO. <br /> • If the injured employee is able to return to the job site the same day, he/she will bring with him/her <br /> a statement from the doctor containing such information as: <br /> o Date <br /> o Employee's name <br /> o Diagnosis <br /> o Date he/she is able to return to work, regular or light duty <br /> o Date he/she is to return to doctor for follow-up appointment, if necessary <br /> o Signature and address of doctor <br /> • As promptly as possible following an injury or illness, ensure appropriate notification has been <br /> made to the family of the individual involved. <br /> • Please see Section 1.6 for incident reporting procedures. <br /> If the injured employee is unable to return to the job site the same day, the employee who transported <br /> him will bring this information back to the job site and report it to the Project Manager, office OSEC, HSSE <br /> MANAGER, Stantec's Practice and Risk Management (PRM -via email to HSSE@stantec.com), and <br /> their regional Human Resources Specialist. <br /> .5.2 Injuries or Illnesses Requiring Hospital Service WITH Ambulance Service <br /> Injuries or illnesses requiring hospital service with ambulance services include severe head injuries, <br /> amputations, heart attacks, heat stroke, etc. The following procedures will be taken immediately: <br /> • Call for ambulance service and notify the SHSO. <br /> • Administer First Aid until ambulance service arrives. <br /> • While the injured employee is being transported, the SHSO will contact the medical facility to be <br /> utilized. <br /> • One designated representative will accompany the injured employee to the medical facility and <br /> remain at the facility until final diagnosis and other relevant information is obtained. <br /> • As promptly as possible following an injury or illness, ensure appropriate notification has been <br /> made to the family of the individual involved. <br /> Stantec <br /> 1.3 <br />
