SITE-SPECIFIC HEALTH AND SAFETY PLAN FOR PHASE II ENVIRONMENTAL SITE ASSESSMENT <br /> Emergency Response <br /> July 7, 2023 <br /> • Please see Section 1.6 for incident reporting procedures. <br /> 1.5.3 Death of an Individual or Hospitalization of Three or More Employees <br /> The emergency response procedures above will be followed. If the injured person dies, follow the <br /> incident reporting procedures. PRM will notify the Human Resources Department, local officials and <br /> coroner immediately. Human Resources will notify the local OSHA office within 8 hours of the incident or <br /> fatality in the event of a fatality or the hospitalization of three or more employees. <br /> 1.6 EMERGENCY RESPONSE PROCEDURES: SPILLS OR CUT <br /> LINES <br /> Prevent problems by documenting the location of underground lines (e.g., product, sewer, telephone, <br /> fiber optic) before starting site work. If a line or tank is drilled through, or another leak occurs, document <br /> the event as soon as possible using the Incident Investigation Report. <br /> In the event of a spill/release, follow this plan: <br /> • Stop Work, stabilize the situation, and secure the site. <br /> • Call Fire Department immediately if the spill cannot be contained or in case of fire or other <br /> emergency. <br /> • Stay upwind of the spill/release. <br /> • Wear appropriate PPE. <br /> • Turn off equipment and other sources of ignition. <br /> • Turn off pumps and shut valves to stop the flow/leak. <br /> • Plug the leak or collect drippings, when possible. <br /> • Use sorbent pads to collect product and impede its flow, if possible. <br /> • Notify the SHSO to begin the incident reporting procedures. All spills/releases will be reported to <br /> the Client Project Manager within 24 hours. <br /> • Determine if the client wants Stantec to repair the damage or if the client will use an emergency <br /> repair contractor. <br /> • Based on agreements, contact emergency spill contractor for containment of free product.
