1.04 Site Specific Hazards <br /> Any unknown undiscovered hazard shall be communicated at the time of discovery to be included Task/Location Hazard Mitigation <br /> immediately into this document New Hazards and Hazards not previously recegnkedshall be addressed upon discovery <br /> Task/Location Hazard Mitigation <br /> Onsite Slip trips and falls Keep a clean level working area,maintain good <br /> housekeeping,and watch footing. <br /> On site Underground utilities Private utility locator will be used to confirm locations <br /> of operating or abandoned utilities and to identify <br /> underground obstructions.Contactor performing <br /> work must possess a cleared USA ticker. <br /> On site Working around Keep a safe distance from drill rig at all times.Utilize <br /> equipment ear plugs to reduce noise exposure in addition to all <br /> required PPE.Avoid areas with potential for vehicles <br /> and drilling equipment to get stuck in soft surface <br /> materials if possible. <br /> Drilling/Boring locations Overhead hazards from Use distance as safety clearance in accordance with <br /> high voltage power OSHA 29 CFR 1926.1407-1411 <br /> lines <br /> General work/On site Fire/Wind/Heat No parkingvehicles on dry brush or have dry brush <br /> hazards from open removed priorto mobilization.Keep water backpack <br /> field and dry conditions on your person,have alternate water source on site, <br /> and keep vehicles equipped with fire extinguisher. <br /> Designate competent person on site as fire watch. <br /> Accidents,Property Utilize spotters as often as necessary for both forward <br /> Driving/On site Damage and reverse operations,seat belts,hands free <br /> (Kleinfelder no device distractions),reduced speeds. <br /> General work/On site Ticks,vermin,animals Be aware of surroundings and wear appropriate <br /> from open field clothing and PPE.Check for ticks throughout the day. <br /> Wash hands before eating/drinking and properly <br /> contain or dispose of any food or beverages to prevent <br /> vermin/animals for entering work site <br /> On Site Peat Soil/Bearing Utilize truck matting for heavy equipment to disperse <br /> mpacityfailure load.Work from reinforced/stabilized soil platforms. <br /> Avoid clearly saturated areas. <br /> On Site Peat Soil/ No idling of unnecessary equipment.Keep water <br /> Combustibility-Fire backpack on your person,have alternate water source <br /> Hazard on site,and keep vehicles equipped with fire <br /> extinguisher.Designate competent person on site as <br /> fire watch.No open flame is anticipated to be required. <br /> Saturate all drummed spoils. <br />
