A=COM <br /> 4.2.3 As applicable, employees shall receive client-required training. <br /> 4.3 Planning <br /> 4.3.1 Health and Safety Plan—At a minimum, a SH&E Plan and task hazard assessments (THAs)shall <br /> be prepared prior to any underground utilities and subsurface installations clearance activities. <br /> • The SH&E Plan will address any required environmental monitoring including gas monitoring, <br /> dust, noise, metals, radiation or other monitoring as may be appropriate for site conditions. <br /> • Employees shall comply with all SH&E Plan requirements. <br /> • The location specific emergency response plan shall be in place, contain procedures <br /> applicable to the potential emergencies presented by the operations, and be reviewed with all <br /> personnel potentially affected. <br /> 4.3.2 S3AM-331-ATT2 Underground Utilities&Subsurface Installation Clearance Flow Chart provides a <br /> summary of the key requirements addressed in this procedure. <br /> 4.3.3 Underground utilities and subsurface installations shall be investigated as being present, including <br /> the following, but not limited to: <br /> • Steam, gas and electric. <br /> • Sewer and water. <br /> • Subterranean tunnels. <br /> • Fibre optics(note: routine geophysical surveys will not identify fibre optic cables). <br /> • Traffic control cables. <br /> 4.3.4 Location of underground utilities and subsurface installations will be confirmed by cross-referencing <br /> available information: <br /> • Maps, as-built drawings and issued for construction (IFC)drawings. <br /> • Plot plans, permits, crossing/encroachment agreements. <br /> • One-Call information, locator and provided surveys. <br /> • Private utility information, locator and provided surveys (e.g. ground penetrating radar(GPR), <br /> electromagnetic, etc.). <br /> • Owner supplied documentation. <br /> • Site walks. <br /> 4.3.5 As applicable, emergency shut-off locations of utilities shall be verified before work activities <br /> commence. <br /> 4.3.6 Jurisdictional, land owner, client and utility owner requirements shall be consulted to determine the <br /> minimum search zone dimensions and appropriate clearance distances. <br /> 4.3.7 As necessary and if possible, adjust locations of excavations or intrusive subsurface work away <br /> from subsurface utilities and installations <br /> 4.3.8 Prior to any excavation or intrusive subsurface work, the S3AM-331-FM1 Underground Utility& <br /> Subsurface Installation Clearance Checklist shall be completed. The form shall be reviewed and <br /> signed by the Manager. <br /> • If the answer to any question in Part 1 of the checklist is"No"or"N/A", no ground disturbance <br /> may take place without review by the Manager, in consultation with SH&E Manager, of the <br /> circumstances related to the particular item.The Manager shall initial beside each"No"or <br /> "N/A"item to indicate review and authorization. <br /> 4.4 Permits, Notifications and Access Agreements <br /> Underground Utilities(S3AM-331-PR1) <br /> Revision 2 September 30,2020 <br /> PRINTED COPIES ARE UNCONTROLLED.
