PMMMR <br /> If the well can be repaired, the damaged portion of the well will be removed/cut-off and new casing with a <br /> slip coupler and neat cement will be used to repair and seal the well. The stovepipe will be re-installed <br /> and a new apron poured to secure it. <br /> If SJCEHD or GHD determines that the well MW-3 cannot be repaired, it will be over-drilled to the total <br /> depth of 150 feet below grade(fbg) using 10-inch hollow-stem augers. Following the drill-out, GHD will <br /> oversee the installation of a new well in the same boring with the same screened interval and well <br /> construction specifications as the previous well. The boring log showing well construction specifications <br /> for MW-3 is included in Attachment B. <br /> 2.5 Well Development <br /> Whether repaired or drilled out and reinstalled, MW-3 will be developed using surge block agitation and <br /> bailer or pump evacuation. A maximum of 10 casing volumes will be purged during the well development <br /> process. Water quality parameters (temperature, pH, conductivity, turbidity, and dissolved oxygen)will be <br /> taken periodically and noted on the well development log. If a new well is installed, development will be <br /> no sooner than 48 hours after placement of the sanitary seal. Water generated during well installation and <br /> development will be placed in drums, labelled and left onsite for future recycling. <br /> 2.6 Wellhead Survey and Groundwater Sampling <br /> Whether repaired or drilled out and reinstalled, the horizontal well coordinates and top of casing for well <br /> MW-3 will be surveyed by a California licensed land surveyor. Groundwater sampling will continue to be <br /> performed as outlined in previous semi-annual monitoring reports and is not included in this scope of <br /> work. <br /> 3. Reporting and Schedule <br /> Upon completion of field activities, GHD will prepare a Well Repair or Well Destruction and Installation <br /> Report that, at a minimum,will contain: <br /> • Descriptions of the well repair or well destruction and installation activities <br /> • San Joaquin County permits <br /> • Boring log <br /> • Well development details <br /> • Well survey data <br /> If the well is over-drilled and re-installed, GHD will also prepare State of California Well Completion Report <br /> and submit it to the Department of Water Resources. <br /> 11205950-RPT-1-Well Repair Work Plan-2019.d= 3 <br />
