October 11,2017 Page 2 of 5 Reference: Remediation Well Destruction Work Plan 3.0 Proposed Scope of Work STTC is proposing to remove forty-eight(48)wells from the Site.The scope of work will include the following: • Obtain all necessary permits from SJCEHD; • Provide a site-specific health and safety plan; • Perform utility clearance of areas surrounding wells to be destroyed; • Air-knife or hand core around each well to be destroyed;and, • Destroy all ozone wells(designated"OS"on the attached Figure 2)by overdrillingl and grouting to the surface per SJCEHD requirements. A summary of the planned field work is presented below. Field and laboratory procedures are provided in Attachment 1. 3.1 HEALTH AND SAFETY PLAN Stantec will provide a site-specific Health and Safety Plan(HASP)for the above scope of work as required by the Occupational Health and Safety Administration(OSHA)standard guidelines(29 CFR 1910.120),and by California Occupational Health and Safety Administration (Cal-OSHA) guidelines (CCR Title 8, Section 5192). Prior to the commencement of fieldwork,the HASP will be updated with appropriate field personnel and subcontractor information. Subcontractors working at the Site will also maintain their own HASP that covers the specific activities they are completing, and will be reviewed by Stantec prior to initiating field activities.The field staff and contractors will review and sign the HASPs before beginning field operations at the Site,and at a minimum daily during field operations. 3.2 PERMITTING Well destruction permits will be obtained from SJCEHD prior to the commencement of field activities. No other agency permitting is anticipated for completion of the proposed work. 3.3 BOREHOLE CLEARANCE ACTIVITIES Stantec will mark the proposed locations with white paint, as required by law, and contact Underground Service Alert(USA)at least one day before a 48-hour review period,prior to the start of boring activities to notify all utility operators in the area to clear the marked locations.
