Rail car door malfunction,causing the potential for the hopper to empty onto the ground. <br /> 2. Silo valve malfunction, causing the entire silo to empty onto the scale and ground. <br /> 3. Malfunction at the load out while loading a truck, causing the entire load out silo to unload onto <br /> the truck,scale and ground. <br /> 4. Truck malfunction, causing the load from a trailer to empty onto the ground. <br /> Emergency Response Equipment Needs: Fly ash poses a dust hazard if a spill occurs,therefore there is <br /> no equipment required to maintain. <br /> Critical Plant Operations Shut Down Or Control: If possible,stop the leak(use water if necessary).All <br /> equipment associated with the leak will be shut down. <br /> Personnel Evacuation and Accounting: All personnel will evacuate the spill area unless the spill is on a <br /> small scale that can be stopped or controlled by personnel.
