The wells were sampled on 12/20/00; See Groundwater Monitoring Well Installation Report dated 04/12/01 submitted by Ground Zero. 06/11/01 Date of EHD correspondence; review of groundwater monitoring well installation report and directive to begin and continue routine quarterly groundwater monitoring and sampling until directed otherwise by the EHD. 06/27/01 Date of CUF letter preapproving corrective costs. 06/28/01 Monitoring and sampling event this date; free product motor oil was discovered in MW-3. The deadline for the work plan submittal was 2/25/02. 03/08/02 On behalf of the Responsible Party, Ground Zero verbally requested an extension until 03/31/02 for submittal of the work plan. 04/04/02 Work Plan for Additional Groundwater Monitoring Well Installation prepared by Ground Zero was submitted to the EHD this date. 04/16/02 Date the EHD conditionally approved the Work Plan, but directed submittal of an addendum to address the issue of vertical delineation. Deadline for submittal of the addendum was 05/13/02. 05/24/02 Date of Revised Work Plan for Additional Groundwater Monitoring Well Installation, two weeks late; it still did not address vertical delineation. 06/11/02 EHD correspondence sent this date denying 05/24/02 work plan because it did not address vertical delineation, and it also changed the number of proposed additional groundwater monitoring wells from three to two; new deadline for submittal of an adequate work plan was 07/08/02. Proposal to conduct a pumping test on MW-3 was approved. 09/06/02 Date of Work Plan for installation of additional wells -two months late.
