Based on the submined work scope, prepared by your consultant, Philip Environmental, letter <br />dated February 14, 1999, pre -approval has been granted for quarterly monitoring of 10 wells ($6,715 <br />per quarter) for one year for an annual cost of $26,860.00. Please note Cost for per diem are not <br />eligible for reimbursement from the Cleanup, Fund Also the Cleanup Fund will not pay for both "truck <br />rental" and "mileage" therefore the Cleanup approved Truck Rental only. All future reimbursement <br />requests must meet the Cleanup Fund's Three Bid Minimum Requirement. This estimate will be placed <br />in your file. <br />You should note that the Reimbursement Request (RR) instructions do not allow for a "lump sum" type <br />invoice submittal. A breakdown by date, time, task and cost accompanied with all subcontractor invoices <br />and work descriptions for each invoice will be required. Consultant/contractor should submit invoices <br />that are consistent in format with the Preapproved cost estimate for timely reimbursement. When <br />unforeseen changes or additions occur, an explanation must accompany the RR or those costs'inay not be <br />reimbursed. Additionally, all work conducted mast be in accordance with all applicable state and local <br />requirements and regulations to be eligible for reimbursement.
