Anderson St., Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I CLASS II,or MINOR-Notice to Com <br /> This report supersedes the "Checklist Only" report left on site on 10/20/2011. <br /> (Violation #108 continues) <br /> Any person who generates a waste shall determine if the waste is a hazardous waste. Hazardous metal <br /> can be recycled only if the particle size is determined to be greater than 100 microns; hazardous metal <br /> particles 100 microns or smaller cannot be recycled and must be handled as hazardous waste. <br /> * Immediately stop sending metal fines to Allied Waste. As a generator you must first make a <br /> hazardous waste and size determination of the metal fines and manage it according the Title 22 <br /> hazardous waste regulations. Immediately make a hazardous waste determination of the above- <br /> mentioned paint filters, paint dusts, absorbents, and metal shavings accordingly, or dispose of the waste <br /> as hazardous waste in accordance to California Code of Regulations (CCR) Title 22. Submit a <br /> statement and supporting documentation explaining how this waste was managed by 11/27/2011. <br /> (Note: EHD has collected a sample from the paint filters and a sample from the metal fines from the <br /> bag-house during this inspection. A split sample of each sample has been provided to you on the same <br /> ddw <br /> 122 & 125. Only one copy of the August 2011 consolidated manifest for used oil, and one copy of the <br /> February 2010 consolidated manifest for used oil and truck wash clean-out (a 3-stage oil/water <br /> separator that flows through to City POTW) were available for inspection. Hazardous waste generators <br /> shall retain copies of all manifests signed off by the disposal facility and all receipts used in a <br /> consolidated manifesting procedure on site for three years and have them readily available for review. <br /> (Please be aware that the maximum time hazardous waste can be stored on site is 6 months; therefore <br /> there should be at least two hazardous waste disposal records per year. Please see Item #701 below) <br /> * Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, <br /> and submit copies to the EH by 11/27/2011. <br /> 126. The February 2010 consolidated manifest missing volume or quantity of the waste from the 3- <br /> stage oil/water separator for the truck wash. All copies of each receipt shall contain all the following <br /> information: (A) Name and address of the generator, and signature of the generator. (B) Date of <br /> shipment. (C) Manifest number. (D) Volume or quantity of each waste stream received, its <br /> California and RCRA waste codes. (E) Name, address, and identification number of the authorized <br /> facility to which the hazardous waste will be transported. (F) Transporter's name, address, and <br /> identification number. (G) The driver's signature. <br /> * Immediately provide a completed copy of February 2010 consolidated manifest (if available) with the <br /> missing volume by 11/27/2011, and begin maintaining future consolidated manifests per this section. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By.
