HSC, section 25160 (b) (3) —Failed to submit TSDF manifest to the Department for <br /> out of state shipments. <br /> Several copies of the manifest page "Designated Facility to Destination State" were <br /> found in the facilities records. Therefore the copy was never sent to DTSC. Provide <br /> documentation that all out of state shipments have a TSDF copy sent to DTSC within <br /> thirty days of receiving the copy. Class I1 Violation <br /> #25. CCR, section 66262.23 (a) (4) — Generator manifest copies not sent to the <br /> Department within thirty days. <br /> No documentation was found that generator copies are being sent to DTSC. Upon <br /> reviewing the Hazardous Waste Tracking System, it was noted that generator copies are <br /> not being received. Provide documentation that that all generator copies are being sent to <br /> DTSC. Class II Violation <br /> #27. CCR, section 66262.40 (a) — Failed to keep sigried copy for three years. Repeat <br /> Violation. <br /> A TSDF manifest copy of manifest number 000412392 GBF from April 23, 2008 could <br /> not be found in the records onsite. Provide a copy of the above noted TSDF manifest and <br /> retain a copy for this facilities records. Class II Violation <br /> #45. CCR, section 66265.52 —Contingency plan incomplete. <br /> Upon reviewing the contingency plan onsite, the plan has personnel that no longer work <br /> for the business. Revise the contingency plan and provide a copy to our department. <br /> Class II Violation <br /> #49. CCR, section 66265.16 — Personnel training records for hazardous waste. Repeat <br /> Violation. <br /> Upon requesting personnel training records for hazardous waste management procedures <br /> the documents could not be found onsite. Mr. Orsolini attempted to fax them to the <br /> business but they were not legible. Provide copies of personnel training records for <br /> hazardous waste management procedures and implementation of the contingency plan for <br /> all relevant employees.
