16.3.3.1 Notify the Production Supervisor of the Emergency. <br /> 16.3.3.2 Meet the ambulance and direct them to the location of the injured <br /> employee. <br /> 16.3.3.3 The Production Supervisor shall note the names of the injured and <br /> their destinations. <br /> 16.3.3.4 Contact the hospital informing them of the number of persons and <br /> types of injuries associated with the incident. <br /> 16.3.3.5 During off-hours, the production supervisor will call the occupational <br /> health coordinator at home, and the safety coordinator (at home). <br /> 16.3.3.6 The Production Supervisor will inform the employee's manager of <br /> the incident. The employee's manager should immediately contact <br /> the employee's immediate family and inform them of the incident. <br /> 16.3.4 If the injury is non-serious, the ERT shall administer appropriate first aid and <br /> determine if additional medical attention is necessary. <br /> 16.3.5 If the injury is non-serious, but the employee requires medical attention, <br /> contact the occupational health coordinator, the safety coordinator, and the <br /> local designated medical facility. <br /> 16.3.5.1 Non-serious injuries can be transported to the hospital by company or <br /> personal vehicle. Another employee should drive an injured employee <br /> to the hospital. Ensure that the employee's supervisor is aware of the <br /> situation. <br /> 16.3.6 Non work related injuries or illness such as flu symptoms, food poisoning, <br /> colds, etc. should be referred to medical treatment according to the <br /> employee's health plan. <br /> 16.4 The injured employee's Supervisor is responsible for initiating an Accident <br /> Investigation Report form. This must be filled out by both the employee and <br /> supervisor. The form should be completed within 24 hours of the incident and <br /> submitted to the Safety Department for follow-up. <br /> 16.5 First aid for contact with hazardous materials must meet the following procedures: <br /> 16.5.1 If there is contact with skin or eyes, immediately direct the employee to the <br /> nearest emergency eyewash/shower. <br /> 16.5.1.1 The employee must stay in the shower a minimum of 15 minutes or until <br /> paramedics take over. <br /> ERT Contingency Plan Page 18 06/23/99 <br />
