17.4 The verbal notification to outside agencies must include the following information: <br /> 17.4.1 Name and phone number of person notifying agency. <br /> 17.4.2 Name of Company and address of facility(s) involved in the emergency. <br /> 17.4.3 Type of emergency (fire, chemical release, injury). <br /> 17.4.4 Identity of chemical(s) and the quantity released, if any. <br /> 17.4.5 Number of injured or ill persons and nature of injuries, if any. <br /> 17.4.6 Possible hazards to the community or environment, if any. <br /> 17.5 For emergencies involving fire: See Fire Procedure Section 12.0 <br /> 17.5.1 Call the Fire Department - 911. Use the general reporting format. <br /> 17.5.2 If there are injuries, see First Aid Procedures Section 16.0 <br /> 17.5.3 If there are chemicals involved, see Chemical Release Section 14.0 <br /> 17.6 For emergencies involving injuries: <br /> 17.6.1 Call for outside emergency services - 911. Use general reporting format . <br /> 17.6.2 Call hospital and notify them that injured employees are being transported to <br /> their facility: <br /> 17.7 For emergencies involving chemical releases: <br /> 17.7.1 Call for outside emergency services - 911. Use the general reporting format. <br /> 17.7.2 Check the CERCLA RQ (Reportable Quantity) list for type of chemical(s) <br /> released and estimated quantity released. Refer to Supervisors Report of <br /> Equipment Breakdown. Emissions, or Spills. <br /> 17.7.2.1 If chemical(s) released is not listed or is less than the Reportable <br /> Quantity, no notification under CERCLA is necessary. <br /> 17.7.2.2 If chemical(s) are listed and the estimated quantity released is equal to <br /> or greater than the Reportable Quantity, notification under CERCLA is <br /> required.
