Manifest number 002407561 SKS from 2010, and from 2009. 000895103SKS, <br /> 001078489SKS, 001773639SKS, 001773640SKS, 001983486SKS, 002151604SKS were found without <br /> a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all <br /> manifests signed off by the destination facility on site for three years and have them readily available for <br /> review. If the generator did not receive a copy of the manifest with the handwritten signature of the <br /> owner or operator of the facility to which the generator's waste was submitted within 35 days of the date <br /> the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the <br /> owner or operator of the designated facility to determine the status of the waste. If the generator did not <br /> receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was <br /> accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible <br /> copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate <br /> the hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. <br /> By August 8, 2011, submit to the EHD a copy of the signed off copy of the manifest or a copy of the <br /> exception report submitted to DTSC. <br /> 63. Copies of hazardous waste consolidated manifest(s) before between November 2009 and April 2010 <br /> were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by <br /> the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years <br /> and have them readily available for review, Immediately locate a copy of all the missing manifests the <br /> last three years and submit copies to the EHD by August 8, 2011 <br /> 79,81,84. Unable to determine the length of storage of approximately 80 universal waste fluorescent <br /> lamps in open containers with no accumulation start date at the storage bay, a bucket of universal waste <br /> alkaline batteries with no accumulation start date in the office, and electronic wastes in a wire container <br /> with open bottoms, sides, and top also without accumulation start date in the storage/repair bay <br /> Universal Wastes shall be stored in a closed container that prevents breakage and release to the <br /> environment, label to identify the type of universal waste, marked with date of accumulation, and <br /> managed within one year of storage.
