CFR 112.3(d) continued from page 5 <br /> • Correct by retaining three years of inspection and test results, signed by the appropriate supervisor or <br /> inspector, with the SPCC Plan. <br /> #32. CFR 112.7(f) Failed to: (1) train personnel on discharge prevention; (2) designate a person <br /> to be accountable for discharge prevention; (3) schedule prevention discharge prevention <br /> briefings at least annually <br /> • Records of training the oil-handling personnel, designated individual and annual schedule prevention were <br /> unavailable for review. <br /> • Oil-handling personnel must be trained in operation and maintenance of equipment to prevent discharges; <br /> discharge procedure protocols; applicable pollution control laws, rules and regulations; general facility <br /> operations; and contents of SPCC Plan. Designated person will be accountable for discharge prevention and <br /> will report to facility management. An annual schedule briefing for your oil-handling personnel is required to <br /> assure adequate understanding of SPCC Plan. Briefings must include highlight and describe known <br /> discharges of potential equipment failures, malfunctioning components, and any recently developed <br /> precautionary measures. <br /> • Correct by providing adequate oil-handling training for your personnel with an annual briefing. Provide in your <br /> SPCC Plan a record of your designated person who is accountable for discharge prevention — preferably <br /> someone who maintains the petroleum equipment and storage. <br /> #33. CFR 112.7(g) Failed to describe how the oil handing, processing and storage areas are <br /> secured and controlled; how master flow and drain valves are secured; how unauthorized <br /> access to starter controls on oil pumps are prevented; how out-of-service and un/loading <br /> connections are secured; the appropriateness of security lighting <br /> • In Section VII, it states that the facility is secured by security guards by an in-house for the day time hours and <br /> outside security service for the weekend and night hours. This section also states it allows the guards for <br /> checking of detection of leaks from containment areas or storage facility. Description of security concerning the <br /> oil-handling, processing, storage, valves, controls, connections for the facility is incomplete. <br /> • Correct by adding a description in your Plan how you would secure and control access to oil-handling, <br /> processing and storage areas; secure master flow and chain; prevent unauthorized access to starter controls <br /> on oil pumps; secure out-of-service and un/loading connection of oil pipelines; and address the <br /> appropriateness of security lighting to prevent both acts of vandalism and assist in the discovery of oil <br /> discharges <br /> #48. CFR 112.8(c)(6) Failed to: test each aboveground container for aboveground container for <br /> integrity on regular schedule or when repairs were made; inspect containers and container <br /> supports; or keep records of inspection and tests <br /> • Records of inspection and test results for aboveground containers were unavailable for review. No records of <br /> inspections and test on aboveground containers were conducted. <br /> • Test or inspect each aboveground container for integrity on a regular schedule and whenever you make <br /> material repairs.
