The generator and waste sections of the uniform manifest shall be <br /> completed and signed and dated by the generator. The transporter shall also sign and date the manifest. Ensure that <br /> all manifests are properly completed for any transport of a hazardous waste for off-site transfer, treatment, storage or <br /> disposal. <br /> This is a minor violation. <br /> 109 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste for the last three years were not found on site. A <br /> single manifest was located on site with a date of June 15th, 2015. Hazardous waste generators shall retain copies of <br /> all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site <br /> for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for <br /> the last three years, maintain them on site, and submit copies to the EHD. <br /> Manifest numbers 005232319JJK (06/15/15), 005232328JJK (6/18/15), and 005232324JJK (6/24/15) were found <br /> without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests <br /> signed off by the destination facility on site for three years and have them readily available for review. If the generator <br /> did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the <br /> generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the <br /> generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received <br /> confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC.
