The manifests for this facility did not use the facilities State ID number(CAL000410682).The manifests are using temporary ID numbers (CAC002818307). The generator and waste sections of the uniform manifest shall be completed and signed and dated by the generator. The transporter shall also sign and date the manifest. Ensure that all manifests are properly completed for any transport of a hazardous waste for off-site transfer, treatment, storage or disposal. This is a minor violation. 109 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. Copies of uniform manifests for the disposal of hazardous waste for the last three years were not found on site. A single manifest was located on site with a date of June 15th, 2015. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. Manifest numbers 005232319JJK (06/15/15), 005232328JJK (6/18/15), and 005232324JJK (6/24/15) were found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC.
