WASTE DISCHARGE REQUIREMENTS ORDER NO. -10- <br /> KEARNEY-NATIONAL,INC. AND UPRIGHT CONSTRUCTION <br /> FORMER KEARNEY-KPF FACILITY <br /> GROUNDWATER TREATMENT AND DISPOSAL SYSTEM <br /> SAN JOAQUIN COUNTY <br /> registration). Each report shall be wet stamped by the registered professional. In addition, the <br /> Discharger shall certify all reports, as required by General Reporting Requirement B.3 of the <br /> Standard Provisions. <br /> a. The Discharger shall submit to the Regional Board and the DTSC the following <br /> reports/documents for approval at least two weeks prior to the commencement of the <br /> performance treatment testing phase of the groundwater treatment system. <br /> i. The Installation Data Report containing a record of the groundwater treatment system <br /> as installed, including completed installation check lists for each component and the 1 <br /> results of the hydrostatic tests conducted on the system's pipelines. <br /> ii. The Quality Assurance Project Plan containing quality assurance and quality control <br /> procedures involved in the collection,preservation,packaging, and transport of <br /> samples, field testing, laboratory analyses, record keeping, data management, and chain- r-n <br /> of custody. 1 <br /> iii. Certifications that the treatment system components are properly installed in <br /> accordance with the design. <br /> b. The Discharger shall submit to the Regional Board and the DTSC the performance <br /> treatment test report for approval at least two weeks prior to prove-out commencement of <br /> the treatment system. This report shall document compliance with Discharge Specification <br /> B.4.c. In addition, this report shall include a performance evaluation of the groundwater <br /> treatment system, based on the data collected during the potable water'start-up phase and <br /> the treatment performance test(in accordance with the attached MRP No. 5-01-XXX). <br /> c. The Discharger shall submit to the Regional Board and the DTSC the prove-out of system T <br /> report for approval at least two weeks prior to the proposed commencement of full scale <br /> operation of the groundwater treatment system. This report shall document compliance <br /> with the Discharge Specification B.4.d. In addition,this report shall include a performance <br /> evaluation of the groundwater treatment system,based on the data collected during the <br /> potable water start-up phase, the treatment performance test, and prove-out of the system T <br /> (in accordance with the attached MRP No. 5-01-XXX). A contingency plan for injection 1 <br /> well(s) failure shall also be included in this report. <br /> Full scale operation shall not commence until the Board determines that the proposed <br /> system will adequately treat all identified pollutants, that the system will comply with <br /> waste discharge requirements,that the treatment system will be operated to maximize VOC <br /> removal, and that its continued operation is appropriate. <br /> E <br />
