0 0 <br />- 4 - <br />• Prevention of off-site releases of hazardous compounds will be <br />achieved by rigorously decontaminating all equipment, sampling <br />containers and other items prior to removing these items from the <br />work site. <br />Any off-site release or suspected off-site releases of hazardous <br />substances must be immediately reported to the Lambert Construction/ <br />Environmental Health and Safety Officer. Any suspected exposure to <br />hazardous substance resulting form PPE failure, accident or other <br />reason must be reported to the Lambert Construction/Environmental <br />Health and Safety Officer, "in writing" with 24 hours. <br />2.0 CONTAMINANT CONTROL <br />Decontamination of sampling equipment so as not cross - <br />contaminate samples is addressed in procedural sections of this <br />document. This section will address the control of contaminants in <br />the interest of health and safety and chemical release prevention. <br />2.1 Decontamination of Heavy Equipment <br />All equipment which may have come in contact with contaminants <br />will be cleaned or otherwise decontaminated on-site within the site. <br />2.2 Personnel Decontamination <br />All employees must thoroughly decontaminate their equipment <br />before exiting the site. <br />3.0 EMERGENCY PROCEDURES <br />3.1 Potential Incidents <br />Situations which could occur but are considered unlikely to <br />occur, that would require an emergency response action are listed <br />below: <br />An explosion or fire occurring during field operations <br />A heavy equipment related or personal injury accident <br />An ambient temperature related injury(ie. hyperthermia or <br />hypothermia) <br />Below is a list of emergency -response agencies and their <br />telephone numbers: <br />General Emergency: <br />911 <br />Fire Department: <br />911 <br />Police Department: <br />911 <br />Emergency Medical Facility: <br />911 <br />Ambulance: <br />911 <br />Environmental Protection Agency <br />(415) 974-8071 <br />National Spill Response Center: <br />(800) 424-8802 <br />Poison Control Center: <br />(800) 342-9293 <br />
