(continued) - hazardous waste management procedures relevant to the positions in which they are employed - contingency plan implementation - emergency procedures, equipment, and systems, including how to maintain and test them - key parameters for automatic waste feed cut-off systems - communications or alarm systems - response to fire, explosion, or groundwater contamination incident - shut down of operations The training program shall be completed by each employee within six months of hire, prior to unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until closure of the facility and shall include: - name &job title of each employee - description for each job title listed, including requisite skill, education, qualifications, and duties - description of training to be given to each position, both initial and as an annual refresher - records that training was completed for each employee Immediately update the hazardous waste management training for this facility and ensure that all employees complete this training within six months of hire and at least annually thereafter. Submit a copy of the updated training records to the EHD by November 18, 2012. 122, 123, & 124. Manifest numbers 008483833JJK (1014111), 008483834JJK (1014/11), and 009479250JJK (5122112) were found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts.
