1 <br /> a <br /> You must submit a map of your facility as part of'your HMMP submission. You can submit hard <br /> copy map(s). Map templates and a sample snap can be downloaded as a PDF file from this <br /> website. If you can scan (digitize) your map(s), you can submit an electronic map file in GIF or b <br /> ]PEG format. For more information on submitting your facility map(s) and downloading a map <br /> template, use the "Preparing and Submitting Your Facility Map" link on the Main Menu of this <br /> website. <br /> I <br /> w <br /> Chemical Description Page <br /> One Chemical Description Page for each hazardous material must be completed and <br /> submitted on this system. The system allows you to create additional Chemical Description <br /> Pages as needed by using the "Add a New Chemical" link from the Main Menu, the inventory <br /> list Table, or existing Chemical Description Pages. You-can also delete a previously submitted <br /> Chemical Description Page by clicking on the checkbox labeled "Delete" at the top of the <br /> { Chemical Description Page you wish to eliminate. When you press "Submit Changes" at the <br /> bottom of the form this Chemical Description Page will be immediately eliminated from your <br /> inventory and a PDF file of the deleted form will be e=mailed to you for reference. To assist you <br /> in completing Chemical Description Forms, an "Information Sheet for Commonly Used <br /> Chemicals" can be accessed from the Main Menu or the Chemical Description Page as you <br /> complete your inventory. <br /> You can submit the same chemical handled at different locations on your facility on one <br /> Chemical Description Page as long as it is-stored in the same physical state. Do not submit <br /> separate Chemical Description Pages for a material that is chemically the same just because it <br /> is stored in different areas. <br /> Account !Numbers and Facility "Records" <br /> When you requested access to the on-line compliance system to meet your submission <br /> q <br /> it re uirements our office created a "record" in the website database for each facility for which <br /> you wanted to complete an Hazardous Materials Management Plan Packet. A unique "account <br /> number" for each facility was assigned to this database "record". This account number along <br /> with the business name for the facility is displayed on the HMMP forms when you log on the <br /> system with that account's user name and password. You must log on separately for each <br /> account that you want to work on. <br /> Viewing Previously Submitted Data and Making Changes <br /> Data previously submitted to our office will be displayed on the forms for the account password <br /> which you used to log onto this website. You can update information as needed on the form <br /> and submit the updated form to our office by selecting the."Submit Changes" button at the <br /> I bottom of the form. An email with a PDF copy of the changed form will be automatically sent to <br /> the business mail address shownon the Main Menu. If no changes are needed, then select the <br /> No Changes Necessary" button at the bottom of the form. You can receive a PDF file of an <br /> existing form even if there are no changes necessary by selecting the Email " form Name" <br /> link at the top of each form. <br /> i - -- <br /> Facility Addresses- -- - -— —— - -- — — <br /> It is important that you provide a numbered address for each separate facility. This allows <br /> public safety agencies to quickly respond to an accident at that location. If you have a facility ! <br /> that does not have an assigned address, e.g. located on a dirt road some distance from a <br /> public road, provide the address of the nearest "Public Access Point". This can be obtained from <br /> the local planning or other department responsible for assigning addresses for the jurisdiction <br /> in which the facility is located. After entering this address for the "Site Address", describe the <br /> location of your facility in relation to this "Public Access Point" in the "Nearest Cross Street" <br /> entry on the Business Owner/Operator Identification Page described above. <br />
