The date and time of the . incident, weather conditions, duration cause, i <br /> environmental problems, response procedures, parties notified, recommended <br /> revisions of the BMP program, operation procedures, and/or equipment needed to <br /> prevent recurrence. <br /> 2. Formal written reports. Document all reports when a call is made to the National <br /> Response Center or State in the event of a reportable quantity discharge. <br /> 3. A list of the procedures for notifying the appropriate plant personnel and the t <br /> names and telephone numbers of responsible employees. h <br /> A <br /> 10.2. Inspection, Monitoring and Maintenance Records <br /> The following records will be kept at the facility. Example inspection logs are provided <br /> in Appendix M. <br /> 1. A log of quarterly area inspections and follow up, as described in Appendix M. <br /> f 2.
