SITE HEALTH & SAFETY PLAN <br /> DIESEL STORAGE TANK ABANDONMENT <br /> 2749 S. ELDORADO STREET <br /> STOCKTON, CALIFORNIA <br /> 1. FACIIdTY BACKGROUND/WORKPLAN <br /> Project consists of the abandonment in place of one underground storage tank. Said tank used to be a <br /> diesel(new product)holding tank for disbursement to vehicles. Testimony indicates that it has been in <br /> disuse for many years. Associated piping shall be decontaminated and removed. <br /> Work tasks include the following: <br /> Ensuring that tank is grounded independently from the existing piping. Rendering the tank inert through <br /> the use of dry ice; Excavation of the surface to determine the outline of the tank; disconnecting any <br /> piping,wiring and appurtenant connections. Inertion of the tank with dry ice. Decontamination of the <br /> tank and piping by an approved contractor. Filling the tank with a concrete slurry. Sampling of the soils <br /> along the piping. Removal of decontaminated piping. Taking the soil/water samples to a laboratory; <br /> Securing the site in a safe manner; <br /> At a later date, if pollutants were found,said pollution shall be handled in a manner approved of by the <br /> Public Health Services department of San Joaquin County. <br /> 2. KEY PERSONNEL and RESPONSIBILITIES <br /> Key personnel will consist of the following people: <br /> A contractor licensed and approved by the San Joaquin County Department of Health to decontaminate <br /> underground fuel storage facilities. Said contractor to be identified prior to start of work. <br /> Ken Willis,owner and operator of heavy construction equipment. Mr. Willis will do the physical removal <br /> himself with the help of his construction equipment. Mr. Willis'address is 321 1'Avenue S.,Pacheco, <br /> CA. 510-687-6244. <br /> Ivan Vegvary,Registered Civil Engineer&Licensed Land Surveyor will do the soil sampling, manifest, <br /> delivery to the laboratory and final report. Mr. Vegvary might also assist in the removal, if needed. <br /> Being that he is an independent contractor-sole proprietor professional,he need not be covered by <br /> workman's compensation insurance.
