The missing alarms include: <br /> 81 T1 Overfill Alarm on 511/17 <br /> 91 T2 Overfill Alarm on 9/7/17 <br /> Diesel T3 Probe Out Alarm on 817117 <br /> Diesel T3 Overfill Alarm on 10/29/17 <br /> Also,the alarm history printouts attached to the June and September 2017 DO inspection reports did not show the <br /> In-Tank Alarm History for T1 (87 tank), T2(91 tank), and T3(diesel tank), and the alarm history print out from the <br /> July 2017 DO inspection report did not show the In-Tank Alarm history for T3(diesel'tank). <br /> During the monthly inspection,the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class ll. <br /> 210 CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. <br /> The designated operator monthly inspection reports from February 2017 through December 2017 were not found on <br /> site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br /> Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br /> maintained on site. Submit copies to the EHD. <br /> Corrected on site. <br /> This is a repeat violation, Class It. <br /> 211 CCR 2715(f)(2) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Ted Shackelford was not current. The designated operator shall <br /> train facility employees for which he or she is responsible in the proper operation and maintenance of the UST <br /> system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4.
