Work will be suspended to correct the cause of the accident/incident and to modify this plan <br /> as necessary. <br /> A WC Health and Safety Incident Report form (Section 10.0) must be submitted to the PM, <br /> HSO/HSC and CMHS within 24 hours of occurrence. The PM will be responsible for <br /> informing the client of any accidents/incidents reported by site personnel. <br /> 6.3 BUDDY SYSTEM <br /> The "buddy system" will be used during all field activities. At no time will an individual <br /> enter or leave an exclusion zone alone. Before entering an exclusion zone each individual <br /> will identify his/her "buddy." <br /> 6.4 PROJECT SAFETY LOGS <br /> Project Safety Log Forms (Section 10.0) shall be maintained by the SSO throughout the <br /> project and turned into the HSC after the project is completed. Logged information shall <br /> include (1) names of all WC Agency and subcontract personnel entering and leaving the site <br /> each day, (2) dates each major field activity started and was completed, (3) air monitoring <br /> data, (4) description of unforeseen hazards and steps taken to mitigate hazards, (5) summary <br /> of telephone conversations regarding health and safety, (6) safety infractions, if any, ( ) <br /> accidents and injuries, and (8) all other significant health and safety items. <br /> 6.5 HEALTH AND SAFETY BRIEFING <br /> Before field work begins, all field personnel, including subcontractor employees, must be <br /> briefed on their work assignments and the provisions of this HSP, and each person briefed <br /> must be given a copy of this document and each must acknowledge receipt and willingness <br /> to comply by submitting a signed safety compliance agreement to the WC Project Manager. <br /> Individuals refusing to sign the agreement will be prohibited from working at the site.
