9.03 Hazardous Materialts461icy I%W <br /> a. chemical and common name of the hazardous chemical; <br /> b. physical and chemical characteristics of the hazardous <br /> chemical; <br /> C. physical hazards of the chemical, including the potential for <br /> fire, explosion and reactivity; <br /> d. health hazards of the chemical, including signs and <br /> symptoms of exposure, and any medical conditions which <br /> are generally recognized as being aggravated by exposure <br /> to the chemical; <br /> e. the primary route of entry <br /> f. OSHA permissible exposure limit, ACGIH Threshold <br /> Limit Value, and any other exposure limit used or <br /> recommended by the manufacturer; <br /> g. whether the hazardous chemical is listed in the National <br /> Toxicology Program Annual Report on Carcinogens, or has <br /> been found to be a potential carcinogen by IARC or by <br /> OSHA; <br /> h. applicable precautions for safe handling and use including <br /> appropriate hygienic practices, protective measures during <br /> repair and maintenance of contaminated equipment, and <br /> procedures for clean-up of spills and leaks; <br /> i. applicable control measures such as engineering controls, <br /> work practices, or personal protective equipment; <br /> j. emergency and first aid procedures <br /> k. the date of preparation of the MSDS <br /> 1. the name, address and telephone number of the chemical <br /> manufacturer or other responsible party who can provide <br /> additional information on the hazardous chemical and <br /> appropriate emergency procedures, if any. <br /> C. Employee Information and Training <br /> 1. Employees will be provided with information and training on <br /> hazardous chemicals in their work area at the time of their initial <br /> assignment and whenever a new hazard is introduced into their <br /> work area. <br /> Pre-emplovment- Upon hire, each new employee will be provided <br /> with a copy of Human Resource Policy 9.03 Hazardous Materials <br /> Policy and will receive general orientation to OSHA Standard <br /> 1910.1200 Hazardous Communication_Upon completion of pre <br /> employment orientation hazardous materials training, each <br /> employee will complete a post-test that will be retained in the <br /> department's Hazardous Materials binder <br /> 3 <br />
