3.0 HEALTH AND SAFETY RESPONSIBILITIES <br /> The following health and safety authorities and responsibilities are established for underground work activities covered under <br /> this HASP. <br /> 3.1 AGE HEALTH AND SAFETY PROJECT MANAGER(Nuel Henderson,Jr.RG) <br /> The AGE Health and Safety Project Manager (H&S PM) has operational management authority for all health and safety <br /> monitoring operations during underground construction as part of this project, and is responsible to the Corporate Health and <br /> Safety Director of AGE for ensuring that all project health and safety activities are completed in accordance with <br /> requirements set forth in this guidance document. The H&S PM is responsible for ensuring that proper health and safety <br /> provisions are incorporated into the management and operation of the work activities. Specific duties include: <br /> • Conducting periodic safety reviews of the project site and project documentation, <br /> • Maintaining compliance with the Health and Safety Plan and other safety regulations, <br /> Approving in writing each addendum to the Health and Safety Plan, <br /> Documenting that site personnel assigned have received the proper training and medical clearance prior to <br /> entering the site, <br /> • Discussing potential health and safety hazards with the designated Health and Safety Professional and the <br /> Site Supervisor. <br /> 3.2 AGE HEALTH AND SAFETY PROFESSIONAL(Michael Noel CIH) <br /> The AGE Health and Safety Professional is a special consultant to AGE designated with coordinating authority for health and <br /> safety activities related to activities where site personnel may come into contact with contaminated soil, water and <br /> construction materials. The Health and Safety Professional is responsible for providing all necessary health and safety-related <br /> technical support necessary for the safe completion of the program.Specific duties include: <br /> • Approval of this HASP, <br /> • Consultation with the H&S PM, Site Supervisor and Site Foreman on the implementation of health and <br /> safety procedures, <br /> Review of health and safety monitoring data, and <br /> • Review of worker health and safety documentation <br /> 3.3 SITE SUPERINTENDENT(Tony Mikacich) <br /> The Site Supervisor is an AGE employee responsible for managing all contractor and subcontractor field activities conducted <br /> during the project. This individual also has administrative responsibility for implementation of the HASP. With regard to <br /> health and safety, this includes communicating site requirements to all personnel, observing that the Contractor(s) Site <br /> Foreman and subcontractors enforce all provisions of the HASP, working with the Site Safety Officer to implement all <br /> elements of this HASP, and consulting with the Health and Safety Professional regarding changes to the HASP in response to <br /> operational changes or improved site knowledge.Other responsibilities include: <br /> • Reading and becoming familiar with this HASP, <br /> • Performing site inspections to find hazards and observe employees at work, <br /> • Ensuring that proper personal protective equipment is available, used properly, and cleaned/stored <br /> appropriately when not in use, <br /> • Stopping work, as required,to maintain personal and environmental health and safety, <br /> • Discussing potential health and safety hazards with the designated Health and Safety Professional and the <br /> AGE H&S PM, and <br /> Page 14 <br />
