Indy Electronics 2 <br /> (1) The EPA identification number, name and address of the generator; <br /> (2) The calendar years covered by the report; <br /> (3) The EPA identification number, name and address for each off-site <br /> treatment, storage or disposal facility to which waste was shipped during <br /> the year; for exported shipments, the report shalt give the name and <br /> address of the foreign.. facility; <br /> (4) The name and EPA identification number of each transporter used <br /> during the reporting year; <br /> (5) A description, California hazardous waste category nunber, DOT <br /> hazard class and quantity of each hazardous waste shipped off site. This <br /> information shalt be listed by EPA identification rxnber of each off-site <br /> facility to which waste was shipped; <br /> (6) The certification signed by the generator or his authorized <br /> representative. <br /> (b) The Department may require generators to furnish additional reports <br /> concerning the quantities and disposition of wastes identified or listed in <br /> Articles 9 and ti of this chapter. <br /> Indy Electronics has not submitted a Biennial Report to this <br /> Department. <br /> 2. section 67105(a) (b) (c) (d) (e) (f) , Title 22, California <br /> Administrative Code <br /> (a) (1) Facility personnel shalt successfully complete a program of <br /> classroom instruction or on-the-job training that teaches them to perform <br /> their duties in a way that ensures the facility's compliance with the <br /> requirements of this chapter. 'rhe owner or operator shall <br /> ensure that this program includes all the elements described <br /> in the document required under subsection (d) (3) of this <br /> sect fon. <br /> (2) This program shall be directed by a parson trained in hazardous <br /> waste management procedures, and shalt include instruction which teaches <br /> facility personnel hazardous waste management procedures (including <br /> contingency plan impt emantation) relevant to the positions in which they <br /> are employed. <br /> (3) At a minimum, the training program shall be designed to ensure that <br /> facility personae( are able to respond effectively to emergencies by , <br /> familiarizing therm with emergency procedures, emergency equipment and <br /> emergency systems, including, where applicable: <br /> (A) Procedures for using, inspecting, repairing and replacing facility <br /> emergency and monitoring equipment. <br /> (3) Key parameter's for automatic waste feed cutoff systems. <br /> (C) Ca mnicatiors or alarm systems. <br /> (D) Response to fires or exptosions- <br />
