The training records did not include the record that the person providing the training (Hafed Majid / Hillary Cole) had current training. The job titles listed for each employee did not match the job titles including with descriptions. The list of required training did not match the training actually provided on site. Facility personnel shall complete a training program that teaches them to perform their duties in a way that ensures the facility's compliance with Title 22 regulations. This program shall be directed by a person trained in hazardous waste management procedures and shall include the following: - hazardous waste management procedures relevant to the positions in which they are employed - contingency plan implementation - emergency procedures, equipment, and systems, including how to maintain and test them - key parameters for automatic waste feed cut-off systems - communications or alarm systems - response to fire, explosion, or groundwater contamination incident - shut down of operations Training records shall be maintained on site until closure of the facility and shall include: - name &job title of each employee - description for each job title listed, including requisite skill, education, qualifications, and duties - description of training to be given to each position, both initial and as an annual refresher - records that training was completed for each employee Immediately update the hazardous waste management training for this facility and ensure that all employees complete this training within six months of hire and at least annually thereafter. Submit a copy of the updated training records to the EHD by January 19, 2013. REPEAT VIOLATION 122, 123, & 124. Manifest number 008642748JJK (2/13/12) was found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
