A copy of the UST operating permit and all attachments <br />and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. <br />This is a repeat violation, Class ll. <br />207 <br />CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br />Unable to determine if the designated operator documented all the alarms, checked and noted the correct test dates, <br />visually check the sumps for leaks, water or debris for the past 12 months and verified all employee has been trained <br />during the last 12 months since the last designated operator, Dan Mcllrath resigned in June 2009. <br />During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br />that each alarm was documented and responded to appropriately, attach a copy of the alarm history with <br />documentation taken in response to any alarms to the monthly report, visually inspect all spill containers and UDC <br />sumps, and any tank top sump that had an alarm for which there is no record of a service visit, check that all required <br />testing and maintenance for the UST system have been completed and document the dates they were done, and verify <br />that all facility employees have been trained. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports and inspecting all of the required areas. <br />This is a repeat violation, Class H. <br />208 <br />CCR 2715 Designated operator monthly inspection reports not maintained on site for at least 12 months. <br />The previous 12 months of the designated operator monthly inspection reports were not found on site. Designated <br />operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that <br />copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. <br />Submit copies to the EHD. <br />This is a repeat violation, Class H. <br />209 <br />CCR 2715(f) Designated operator employee training not performed or log not kept. <br />Records of the designated operator employee training for all employees was not performed and/or not found on site. <br />The designated operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4.
