ACCIDENT, INJURY AND ILLNESS INVESTIGATIONS (8 CCR 3203 (a) (5) ) <br /> * When accidents, injuries, or illnesses occur on the job <br /> the Foreman on the job will complete an "Illness and <br /> Accident Investigation Report" . This report is to be <br /> turned into the Construction Superintendant within 24 <br /> hours and the Superintendant will forward this to the <br /> Safety Committee within eight hours of receipt. <br /> * When accidents, injuries, or illnesses occur on the job <br /> which require medical care, they will be thoroughly <br /> investigated by a member of the Safety Committee. <br /> Investigators will complete the Accident Investigation Form which <br /> is included in this manual as Appendix B. <br /> IIPP. The investigation will determine at least the following: <br /> a. Who and What was directly involved in the accident. <br /> b. Who and What was indirectly involved in the accident. <br /> c. Where and When the accident occurred. <br /> d. The Cause of the accident, if known. <br /> e. Steps/Procedures to take to prevent re-occurrence. <br /> METHODS AND PROCEDURES FOR CORRECTING UNSAFE OR UNHEALTHY <br /> CONDITIONS, WORK PRACTICES (8 CCR 3203 (a) (6) ) <br /> 1. All unsafe or unhealthy work conditions or work practices <br /> identified will be evaluated and corrected. <br /> 2 . Unsafe or unhealthy work conditions or work practices will be <br /> corrected in a timely manner, as determined by the severity of <br /> the hazard. Under no conditions will FHEMCO personnel be <br /> required to, or permitted to, work under conditions which pose <br /> a clear or imminent hazard. <br /> 3 . Problems that cannot be corrected immediately, will be <br /> assigned to the Responsible Person to ensure completion of <br /> the corrective action. Once corrected, written documentation <br /> of the action taken will be developed or obtained by the <br /> Responsible Person. <br /> 4 . When an imminent hazard exists which cannot be immediately <br /> corrected without endangering employees and/or property, the <br /> following steps will be followed; <br /> a. Remove all potentially endangered employees; <br /> b. Provide employees responsible to correct the condition <br /> with necessary safeguards; <br /> c. Correct the problem; <br /> 8 <br />
